For your convenience, we have made a number of forms available for new clients to download.    If you are new to our practice, we encourage you to download and print out the new client forms.

Filling these out in advance of your first visit will minimize the time you spend in the waiting room filling them out when you get here.

If you are coming for individual counseling, you will need:

• New Client Packet

If you are coming for couples counseling, you will need

• New Client Packet for Couples
• Couples Consent Form

For adolescent counseling, you will need:

• Adolescent Intake Form (for adoelscent clients to fill out)
• Parental Consent Form (Parental consent for treatment)
• Parental Agreement (For confidentiality)

There are two ways to setup an appointment with one of our counselors:
